Community Connector Magazine is a free 'physical' magazine designed to support patients and communities by tackling digital exclusion and providing clear, accessible information about local health and wellbeing services and community events. 


The magazine includes an A–Z guide covering NHS services, community support, charitable organisations, and healthcare providers across your locality and the surrounding region. Our aim is to help patients better understand what services are available, how to access them, and where to find further support.
